Título: Unusual occurrence of actinomycosis in dorsum posterior of the tongue

Resumo: Actinomycosis of the tongue is a rare suppurative infection, oftenmimicking benign or malignant neoplasms. The diagnosis of oral actinomycosis can bechallenging. A 48-year-old male presented with a 2.5 cm ulcer in posterior dorsum of thetongue, with indurated borders, a necrotic center and a rapid growth over 30 days wasobserved. The patient showing dysphagia. Clinical diagnosis was squamous cell carcinoma.An incisional biopsy was performed, and the material sent for histopathological analysis.Microscopically, an extensive abscess with polymorphonuclear leukocytes and fibrinsurrounded filamentous bacterial colonies in a radiating pattern, suggestive of Actinomycesisraelli was observed. The bacterial colonies showed positivity for PAS and Brown Brenn stainings and Grocott Methenamine Silver staining was negative. Additionally, the oral mucosa consisting of parakeratinized squamous epithelium withpseudoepitheliomatous hyperplasia and PMN exocytosis, and a dense inflammatory infiltrate with sulfur granules in connective tissue were also observed. The patient wasreferred to an infectious physician and achieved complete lesion remission after treatment
