Resumo: Kaposis Sarcoma is a rare vascular neoplasm caused by infection with HHV8, that is prone to developing in HIV-infected individuals. A 51-year-old male patient hospitalized at a reference hospital for tropical diseases presented a lesion on the dorsum of the tongue, a well-delimited reddish plaque with a smooth surface with petechiae on it, a well-adhered lesion surrounded by an extensive depapillate region. The patient presented with HIV, CMV, and tuberculosis infections and was on regular antiretroviral therapy. Preoperative exams included CBC and INR. Two lesion sites were collected by incisional biopsy with a sample sent for histopathological analysis, the diagnostic hypothesis of CMV. Histopathological analysis with IHC showed CMV-, EBV+, D2-40+, HHV-8 focally positive, and Ki67+ (60%) confirmed the diagnosis of Kaposis Sarcoma. His case is under post-diagnosis follow-up with regression of the lesion.
