Resumo: Immune-mediated diseases can present clinical manifestations with different severity degrees. Two cases of paraneoplastic pemphigus will be described. An 85-year-old woman sought emergency care for severe pain, blisters, and ulcerations on her skin and mucous membranes following pharyngotonsillitis treated with antibiotics. Differential diagnoses included erythema multiforme and pemphigus. A 51-year-old man presented with pruritic skin lesions, oral ulcers, and erythematous areas in the oropharynx, impairing feeding, with clinical suspicion of pemphigus vulgaris. Both patients underwent biopsies and laser therapy for pain control and biostimulation. Histopathology confirmed pemphigus with subtype to be clarified in the first case, and was compatible with pemphigus vulgaris in the second. Hematological evaluation and immunofluorescence revealed monoclonal gammopathy of undetermined significance in both patients, leading to the diagnosis of paraneoplastic pemphigus. These cases highlight the importance of a comprehensive diagnostic approach, including biopsy, immunofluorescence, and hematological evaluation, for accurate diagnosis and management of these complex conditions.
