Resumo: Introduction: Herpes simplex is a common viral infection caused by human herpesviruses, especially type 1 (HSV-1), which affects the oral, facial, and ocular regions. The virus has a DNA core, icosahedral capsid, tegument, and a lipid envelope with viral glycoproteins. Reactivation may be triggered by stress, immunosuppression, and pregnancy, with cold sores being the most frequent presentation. Case Report: A male patient, 1 year and 8 months old, presented with swelling of the lower eyelid. Conjunctivitis was initially suspected, but examination revealed vesicles, ulcerations, and partial eyelash loss. His mother reported recurrent cold sores. The diagnosis was ocular herpes (HSV-1). Treatment included oral and topical Acyclovir for 15 days. Recurrences occurred every 6-8 months. At age 8, the condition evolved into cold sores, suggesting reinfection. The last ocular episode occurred at age 13, with only the labial form recurring biennially. Acyclovir was maintained during prodromal periods.
