Resumo: Adenoid cystic carcinoma (ACC) is one of the most common types of malignant salivary gland tumors. However, cases involving the mandible are rare. We present two cases of ACC with mandibular involvement. The first case is a 74-year-old woman with a right-sided painless mass in the mandible that had been growing for a year. Imaging showed an extensive multilocular radiolucent area involving the mandible and maxilla with irregular shape and cortical bone resorption, ACC was confirmed by an incisional biopsy. The second case is a 57-year-old woman with a nodule on the anterior mandible alveolar ridge, misdiagnosed as a radicular cyst, which did not resolve after tooth extraction. Imaging revealed diffuse resorption around teeth 33 and 42, and an incisional biopsy confirmed ACC. These cases illustrate that clinical and imaging findings of ACC with bone involvement can lead to misdiagnosis, highlighting the importance of biopsies when malignancy is suspected.
