Resumo: Langerhans cell histiocytosis (LCH) is a proliferation of immature myeloid dendritic cells from the bone marrow, leading to their accumulation in various tissues and organs with diverse clinical manifestations. It often affects the oral cavity, and is more common in early childhood. A 5-month-old female patient was referred to the pediatric emergency room due to a palatal wound that hindered breastfeeding. An extraoral examination revealed erythematous papular skin eruptions, previously misdiagnosed as atopic dermatitis, which had not improved with treatment. Intraoral examination showed bilateral ulcerations in the upper alveolar ridge. The primary diagnostic hypothesis was LCH, confirmed by biopsies of the oral and skin lesions. The patient received chemotherapy but ultimately passed away due to disease complications. LCH is a rare condition that can be fatal if untreated, especially with systemic involvement. Early diagnosis is essential for improving prognosis, particularly in pediatric patients, as it can mimic other lesions.
