Resumo: With the growth of hospital dentistry, the role of dentists in Intensive Care Units (ICUs) is increasingly recognized, especially in preventing respiratory infections. However, the influence of oral health on the outcomes of oncology patients in ICUs remains underexplored. This prospective, observational, cross-sectional study aimed to evaluate the oral health of cancer patients admitted to an ICU and its impact on clinical outcomes.A total of 234 adult patients admitted for unplanned hospitalizations were assessed in three phases: within 24 hours of ICU admission, on day 3, and day 5. Evaluations included clinical performance (ECOG), comorbidity index (CCI), and oral assessments such as intraoral exams, salivary pH, oral lesion analysis, and Bedside Oral Examination (BOE) scores.Of the participants (129 women, 105 men), 81.2% had oral alterations requiring intervention. The average BOE score decreased over time (13.4 to 12.52; p = 0.001). Patients needing intervention had higher CCI scores (p = 0.007). Oral issues were significantly associated with metastatic tumors (p = 0.018), disease progression (p = 0.030), and xerostomia grade II/III (p = 0.001). The main reasons for ICU admission were hypoxemic respiratory failure, septic shock, and sepsis. Findings underscore the need for targeted oral care protocols for ICU oncology patients.
