Resumo: Orthokeratinized odontogenic cysts (OOCs) are rare lesions, comprising about 1% of all odontogenic cysts. It is most common in young males (3:1), with peak incidence during third and fourth decades. Most cases involve the mandible (90%), with multiple and bilateral cases being rarely reported. Keratinizing odontogenic cyst with verrucous pattern represents an uncommon and heterogeneous odontogenic cystic lesion subgroup, with some of these cases meeting diagnostic criteria for OCC. Moreover, malignant transformation of OCC has also been reported. A 46-year-old male was referred for evaluation of a radiolucent lesion in the anterior maxilla. The adjacent teeth were vital. On CBCT the lesion exhibited defined borders and cortical expansion. Aspiration yielded cystic fluid content. After enucleation of the lesion, microscopy revealed OOC with multifocal areas showing verruciform morphological pattern. HPV, both by immunohistochemistry and in situ hybridization, was negative. An 8-month postoperative panoramic radiograph showed bone regeneration at the lesional site.
