Resumo: The present work aims to report a 71 years old female patient who was referred to the oral medicine department presenting recurrent multiples painful ulcers in right buccal mucosa and ventral surface of the tongue, associated with bilateral non detachable white plaques in the same areas. The patient reported pain remission with use of prednisone in past episodes. Medical history disclosed previous sigmoid cancer treated by chemotherapy. Incisional biopsy revealed orthokeratinized stratified epithelium presenting irregular stratification, drop-shaped rete ridges and three-tier cytological signs of dysplasia, besides viral cytopathic alterations suggesting cytomegaloviruses. Further immune histochemical investigation showed positivity to Ki 67 (25%), panCK, P53, EBV and negativity to CMV. So, the patient was referred to a reference center with diagnosis of in situ squamous carcinoma associated with moderate EBV related Dysplasia. Excisional surgery with free margins revealed Verrucous Dysplasia. Currently, she is under nine months follow-up with no signs of recurrence.
