Resumo: Oral melanoma is a rare malignant neoplasm, and its clinical presentation can be challenging. A 45-year-old male patient presented with a 3-month history of an increased volume in the attached gingiva of the left upper first premolar. The lesion was painless, measuring approximately 2 cm in diameter, and bled upon palpation. Due to these features, the clinical diagnosis of pyogenic granuloma was made, and an excisional biopsy was performed. Under histopathological analysis, the species revealed neoplasm proliferation composed of spindle cells associated with melanin production. Moreover, immunohistochemical stains revealed positive for S100, HMB45, and Melan-A, confirming the diagnosis of melanoma. The patient was subsequently referred to the head and neck surgery service, where he underwent a maxillectomy. He has been under follow-up, free of disease, for 40 months. This study highlights the importance of microscopic analysis in oral pathologies, where malignant neoplasias can be misdiagnosed as benign processes.
