Resumo: Mucoepidermoid carcinoma is a malignant neoplasm of salivary glands, especially uncommon in the tongue region. This study reports the case of a 25-year-old female patient whose main complaint was a sensation of "shock" on the right side of the tongue, which began after a ulectomy five years prior. Clinical examination revealed a nodular lesion at the base of the tongue, with a telangiectatic surface, firm on palpation and associated with pain. Magnetic resonance imaging showed a well-defined lesion measuring 1.0 cm, suggesting possible neural origin. Surgical excision was performed, followed by histopathological and immunohistochemical analysis. Histopathology revealed a predominantly solid neoplasm composed of squamous, mucous and intermediate cells. Immunohistochemistry was positive for P63 and cytokeratins 40, 48, and 50, establishing the diagnosis of intermediate-grade mucoepidermoid carcinoma. A second surgery was performed to enlarge the margins and remove the involved lymph node chains. The patient remains under oncological follow-up.
