Resumo: Odontogenic carcinoma with dentinoid (OCD) is a rare and controversial entity and not yet recognized as an independent entity in the latest WHO classification of 2022 A 61-year-old brown male patient presented to the Oral and Maxillofacial Service complaining of increased volume in the left mandibular branch. The slow-growing lesion was in the region of the cheek mucosa and the ramus of the left mandible, without bleeding and ulcerations. Radiographic, panoramic and tomography examinations were observed. well-defined radiolucent and radiopaque image suggestive of Calcifantde Odontogenic Tumor. An incisional biopsy was performed and the surgical specimen was sent for anatomopathological diagnosis. The final report was odontogenic carcinoma with dentinoid. The patient was referred to the Oncology Center for treatment. OCD is a locally aggressive low-grade malignancy with no apparent metastatic potential. The wide surgical excision with clear margins and long-term follow-up to identify possible recurrences or metastases is recommended.
