Resumo: Amyloidosis is a rare disease characterized by the accumulation of abnormal proteins in tissues. Systemic amyloidosis is often associated with underlying diseases. We report the case of a patient with macroglossia and granulomatous nodules with yellowish pearls on the tongue and buccal mucosa, accompanied by burning sensations and muscle retraction symptoms. Systemic symptoms included swollen hands with non-pitting edema, a "shoulder pad sign" lesion, and scleroderma-like lesions on the chest. An incisional biopsy of the tongue showed Congo-red greenish-yellow amorphous hyaline material in the connective tissue under polarized light, confirming the diagnosis of amyloidosis. The patient was referred to a rheumatologist. Proteomic analysis revealed light-chain (AL) amyloidosis subtype but no underlying cause was identified after thorough systemic investigation. The patient started chemotherapy treatment and is under close follow-up. This case describes a rare case of idiopathic primary amyloidosis diagnosed through oral manifestations.
