Resumo: A 56-year-old female patient was referred for dental assessment due to facial asymmetry and epistaxis. Clinical examination revealed an increase in volume in the middle third of the face on the right, associated with proptosis. Intraoral examination revealed a tumor in the maxilla, in the region of the molars. A computerized tomography scan was then performed, which showed an expansive lesion in the maxillary sinus, invading the orbital floor and palate. Based on these exams, an incisional biopsy was performed under local anesthesia, and the material was sent for histological analysis, which showed proliferation of spindle cells. Immunohistochemical markers were negative for S100, MYOD1, HMB45, AE1/3, myogenin, desmin and CAM5.4, and positive for vimentin and Ki67 (80%). Based on the immunohistochemical pattern, the diagnosis was fibrosarcoma. The patient then underwent hemimaxillectomy and neck dissection, followed by 34 sessions of radiotherapy (75 Gy). After 26 months of treatment, she is disease-free.
