Resumo: A 46-year-old woman presented to the CTBMF department with mild swelling in the lower facial third, progressively worsening over 15 years. Physical examination revealed increased volume in the right buccal region and posterior mandibular vestibule. Preoperative imaging (including facial computed tomography [CT]) demonstrated cortical bone rupture, mandibular canal displacement, and hypodense areas with extracortical extension, suggestive of aggressive behavior. An incisional biopsy confirmed multicystic ameloblastoma with a plexiform growth pattern on histopathologic examination. The patient underwent partial mandibulectomy with reconstruction using a titanium mandibular plate. Postoperatively, she was in stable condition with persistent right lower facial paresthesia. Clinical follow-up was chosen. Early intervention and tailored management are critical for optimizing outcomes in benign and malignant head and neck lesions.
