Resumo: A 10-year-old boy was referred for the diagnosis of multiple corrugated and non-removable white plaques throughout the oral mucosa with one year duration. No lesions were observed on the nails, skin, or other mucosal areas, and no similar findings were noted in his immediate family members. An incisional biopsy of the mandibular buccal sulcus revealed hyperplastic stratified squamous epithelium with acanthosis, hyperkeratosis, and vacuolated keratinocytes with perinuclear eosinophilic condensation, confirming the clinical hypothesis of White Sponge Nevus. The patient was treated with 0.25% tetracycline mouthwash, twice daily, achieving an 80% improvement in six weeks and remains under follow-up. White Sponge Nevus, or Hereditary Spongy Leukokeratosis, is a rare autosomal dominant genodermatosis with variable penetrance and expressivity. The differential diagnosis, particularly in adults, should include potentially malignant white lesions, and topical tetracycline offers a cost-effective option when plaques cause discomfort.
