Resumo: The Regional Odontodysplasia is associated with the indistinctness of enamel and dentin tissues, non-crossing of the midline and can affect both deciduous and permanent teeth, mainly affecting the maxilla. Male patient, 14 years old, non-white, was referred to the stomatology clinic by the orthodontist because of a radiographic finding. During the intraoral examination were observed gingival hyperplasia, malformation of elements 31, 33, 36, abscess formation in the anterior teeth and absence of element 32. No noteworthy changes were observed in the extraoral examination. Panoramic and periapical radiographs revealed low enamel and dentin density, anomalous roots and periapical lesions. Extraction of the teeth involved and enucleation of the lesions was performed. The histopathology report revealed Radicular Cyst and Regional Odontodysplasia. The patient continues oral rehabilitation treatment in order to re-establish function and aesthetics. The diagnosis is complex and can only be made by associating clinical, radiographic and histopathological aspects.
