Título: Nodular fasciitis in oral mucosa: a rare entity

Resumo: A 57-year-old male patient presented to the Oral Medicine clinic with an asymptomatic 1.5 cm submucosal nodule on the left buccal mucosa, with normal overlying mucosa, and a one-month history. His medical history included Dutam use for benign prostate enlargement and a 30-year smoking history, which he quit a decade ago. The differential diagnosis included benign mesenchymal or salivary gland tumors, prompting an excisional biopsy. Histopathological analysis revealed a non-encapsulated spindle cell proliferation infiltrating skeletal muscle fibers. The cells were arranged in fascicular or loose patterns, exhibited mild pleomorphism, and had vesicular to slightly hyperchromatic nuclei, with focal areas suggestive of early necrosis. Immunohistochemistry showed strong SMA positivity, while S100, CD34, HHF-35, beta-catenin, and desmin were negative, supporting a diagnosis of nodular fasciitis-a self-limiting myofibroblastic proliferation that may mimic malignancy. FISH for USP6 gene rearrangement was unavailable to confirm fusion status. The patient remains under close clinical follow-up.
