Resumo: The present work aims to report a 51 years old female who was referred to the oral medicine department presenting macrocheilia in lower lip associated with a 1,5-year painful ulcer. Medical history revealed negative small bowel biopsy for granulomatous infection and weight loss, but no cough. Excisional biopsy was performed revealing granulomatous tissue presenting diffuse linfocitary infiltrate, besides macrophages and giant multinucleated cells granulomas containing small round bodies Ziehl Neelsen (ZN) staining negative and Giemsa staining positive. A chest x-ray was requested, which showed no signs of alterations. So, the patient was referred to a reference center with compatible diagnosis of buccal trypanosomiasis to further serological investigation, which was negative for Chagas Disease (twice), HIV, Syphillis and positive for Tuberculosis, corroborated by CT presenting "tree in bud" pattern. A new negative ZN biopsy was performed. The patient was successfully treated with antituberculous therapy. Currently, she is under one year follow-up.
