Resumo: A 32-year-old male patient was referred to the oral diagnostic service with a lesion on his lip. The patient reported 4 months of evolution and was diagnosed with HIV. The intraoral physical examination revealed a purplish papule with an erythematous halo, of fibrous consistency, mobile on palpation and negative on diascopy. Given these characteristics, the diagnostic hypothesis was vascular thrombus. An excisional biopsy was performed and the material was sent for anatomopathological examination, in which a fragment of oral mucosa covered by stratified squamous epithelium exhibiting nuclear hyperchromatism and perinuclear vacuolization, moderate cellular pleomorphism, and teardrop-shaped epithelial projections was observed. An immunohistochemical reaction for the p16 protein was performed, which was positive in the epithelial cells, concluding the diagnosis of severe epithelial dysplasia associated with HPV. The patient has been under clinical follow-up and has not shown signs of recurrence for 11 months.
