Título: ALK+ ANAPLASTIC LARGE CELL LYMPHOMA OF THE MANDIBLE IN A PEDIATRIC PATIENT

Resumo: ALK-positive anaplastic large cell lymphoma (ALCL) is a CD30-positive mature T-cell lymphoma with aberrant ALK expression, commonly involving lymph nodes (90%). Young males are more frequently affected. ALK+ ALCL accounts for approximately 12% and 3% of pediatric/adolescent and adult non-Hodgkin lymphoma cases, respectively, with long-term survival rates around 80%. Intraoral involvement by ALK+ ALCL is rare; to date, fewer than five cases have been reported. We report a case of a 12-year-old boy who presented with a progressively enlarging exophytic mandibular mass in late 2024. Radiographs revealed bone infiltration. Microscopy showed solid proliferation of anaplastic large cells, immunopositive for ALK, EMA, CD4, CD25, CD30, and perforin, and focally positive for CD45RB, BCL-2, HLA-DR, and CD99. The Ki-67 index was 75%, and EBER1/2 was negative. CT, MRI, and bone scintigraphy showed systemic involvement. Chemotherapy was initiated. ALCL should be considered in the differential diagnosis of intraoral tumor masses in pediatric patients.
