Título: CONSERVATIVE SURGICAL APPROACH OF CEMENTO-OSSIFYING FIBROMA IN CONTACT WITH THE INFERIOR ALVEOLAR NERVE: CASE REPORT

Resumo: Ossifying fibroma is a benign, slow-growing neoplasm composed of fibrocellular tissue with bone or cementum-like formation. This case describes a 34-year-old asymptomatic female patient who presented with a recurrent mandibular lesion initially identified on a panoramic radiograph taken for orthodontic purposes. Following an unsuccessful previous excision, new imaging studies, including computed tomography and bone scintigraphy, revealed an expansive lesion in the right mandibular body, in close contact with the inferior alveolar nerve. The surgical approach included submandibular access, creation of a vestibular bone window, complete enucleation, curettage, and peripheral osteotomy, preserving the nerve. Histopathological analysis confirmed a cemento-ossifying fibroma. The conservative strategy enabled proper visualization and nerve protection throughout the procedure, with a satisfactory clinical outcome after two years. This case emphasizes the importance of early diagnosis, careful use of complementary imaging exams, and long-term follow-up given the recurrence potential of fibro-osseous lesions.
