Resumo: Adenocarcinoma not otherwise specified (ACNOS) is a rare malignant tumor of the salivary glands that requires detailed microscopic evaluation and exclusion of other entities for an accurate diagnosis. A 42-year-old female patient presented to the dental service with a normochromic, smooth-surfaced nodule on the ventral surface of the tongue, measuring 15 mm, with a two-month history. The clinical differential diagnosis initially suggested a reactive lesion. After biopsy, microscopic examination revealed a malignant neoplasm of glandular origin, characterized by columnar epithelial cells exhibiting nuclear pleomorphism. Ductal differentiation and cystic areas were also observed. Immunohistochemical analysis showed tumor cell positivity for CD117, AE1/AE3, and CK7, while p40, p63, GATA3, and S100 were negative. Ki-67 expression was detected in 10% of the tumor cells. A definitive diagnosis of ACNOS was established. The patient was subsequently referred to a cancer center and remains under clinical follow-up.
