Resumo: The calcifying epithelial odontogenic tumor (CEOT), or Pindborg tumor, is a benign but locally aggressive neoplasm. We report the case of a 44-year-old male who presented with a progressive swelling in the middle third of the face over one-year. An increased volume was observed in the right vestibule, with obliteration of the sulcus. The CT scan revealed a mixed lesion with a thick hyperdense border and a hypodense center, with expansion into the maxillary sinus, nasal cavity, and hard palate. The incisional biopsy revealed islands, nests, and cords of odontogenic epithelium, ghost cells, and dentinoid material deposition within a connective tissue stroma. The lesion was excised via a Weber-Fergusson approach for partial hemimaxillectomy, followed by maxillary contour reconstruction with a titanium mesh. At the six-month follow-up, the patient showed no signs of recurrence and had preserved masticatory function.
