Resumo: Objective: Low-grade myofibroblastic sarcoma is a rare intermediate-grade malignant neoplasm, rarely metastasizing but with high rates of local recurrence. The lesion has variable anatomical distribution and shows a predilection for the head and neck region. The aim of this study is to report a case of low-grade myofibroblastic sarcoma. Case Report: A 30-year-old female patient presented with a painless nodule on the middle third of the left side of the tongue. The lesion was firm on palpation, well-defined, measured approximately 8 mm in diameter, had an intact epithelial surface, and had evolved over 30 days. An incisional biopsy was performed, but the histopathological analysis was inconclusive. Immunohistochemical staining showed positivity for smooth muscle actin, indicating myofibroblastic differentiation. The diagnosis was low-grade myofibroblastic sarcoma. The patient underwent surgical resection of the tumor with clear margins and remains under follow-up at the oncology center, with no recurrence after 2 years.
