Resumo: The Squamous Odontogenic Tumor (SOT) may occur in association with a Radicular Cyst, developing secondarily from its epithelial lining, as demonstrated in the following case. A 61-year-old male patient presented to the Oral and Maxillofacial Surgery Residency Service following an imaging examination for implant-based rehabilitation. This examination revealed a well-defined radiolucent lesion, painless, with no buccal cortical expansion and with a three-year evolution, located in the mandibular body region. Clinically, the overlying mucosa appeared normal and teeth 42-45 exhibited unsatisfactory endodontic treatment, with tooth 44 showing dark discoloration. Initially, a FNAC procedure was performed, with preliminary diagnosis suggesting Mucoepidermoid Carcinoma. However, incisional biopsy was later performed along with the extraction of teeth 44 and 45, removing the capsular-like tissue between these teeth. The final diagnosis was Squamous Odontogenic Tumor associated with a Radicular Cyst. The patient remains under follow-up with no signs of recurrence to date.
