Resumo: A 52-year-old white woman was referred for evaluation due to the presence of a lesion on the lower lip. The patient denied a history of smoking or chronic sun exposure but reported occasional alcohol consumption. Clinical examination revealed a firm, 10-mm lesion with brownish-reddish coloration, which exhibited bleeding upon manipulation. Patient reported that the lesion had been present for one year, emerging shortly after orthodontic appliance placement. Given the clinical hypothesis of pyogenic granuloma, an excisional biopsy was performed. Microscopic examination confirmed a diagnosis of superficially invasive SCC, with neoplastic cells infiltrating the lamina propria. The patient was informed of the diagnosis and referred to a specialized oncology center for further management. This case highlights the importance of considering SCC in the differential diagnosis of persistent lip lesions, even without classical risk factors. Early biopsy and histopathological evaluation are crucial for accurate diagnosis and timely intervention.
