Título: ADENOID CYSTIC CARCINOMA OF THE HARD PALATE MISDIAGNOSED AS AN ODONTOGENIC LESION: A CASE REPORT

Resumo: This study reports a case of adenoid cystic carcinoma (ACC) in the hard palate diagnosed at an advanced stage, after initially being misinterpreted as a benign odontogenic lesion. A 47-year-old woman noticed slight swelling in the hard palate. After two years, lesion growth and mucosal ulceration prompted her to seek dental care. A radiograph was taken, and, assuming an odontogenic lesion, teeth 27 and 28 were extracted. The lesion then grew significantly, prompting referral for Oral and Maxillofacial Surgery. Clinical examination revealed extraoral swelling in the left malar region, a 6.4 cm bulging mass in the hard palate, and mucosal ulceration. An incisional biopsy confirmed solid ACC. Imaging exams identified pulmonary metastasis. Surgical resection, cervical dissection, and chemotherapy were performed, and the patient passed away 17 months after diagnosis. This case highlights the importance of early and accurate diagnosis in suspected oral cavity lesions.
