Resumo: Polymorphous adenocarcinoma (PAC) is a rare malignant salivary gland neoplasm that arises almost exclusively in the minor salivary glands. An 80-year-old male patient, with no history of drug abuse, presented to a primary healthcare unit with an asymptomatic abscess in the hard palate envolving over five years. Intraoral clinical examination revealed a bluish, circular swelling with a soft-fibrous consistency. Fine-needle aspiration suggested a solid content. An incisional biopsy was performed under the initial diagnostic hypothesis of a benign salivary gland tumor. Histopathological and histochemical analysis with mucicarmine and PAS, along with IHC showing: Ki67+ (20%), P63+, P40+, S100+, and CK7+, confirmed the diagnosis of PAC, representing a low-grade malignant lesion. The patient was referred to an oncology service for excisional biopsy and has been under clinical follow-up for 1 year and 5 months with no signs of recurrence.
