Resumo: This study aims to describe the clinical and histopathological features of a primary oral leiomyosarcoma. A 60-year-old woman presented with a vegetative and ulcerated lesion in the floor of the mouth, measuring approximately 7 cm in greatest dimension, with a necrotic center, reports of pain and spontaneous bleeding, with a two-month history. The lesion exhibited bone involvement and no evidence of metastasis. Histopathological examination revealed an ulcerated spindle cell neoplasm with moderate cellular pleomorphism and atypical mitotic figures. Immunohistochemical analysis was positive for vimentin, desmin, αSMA, and h-caldesmon, and negative for cytokeratins. The Ki-67 cellular proliferation index was positive in approximately 25% of the neoplastic cell population. Treatment consisted of surgical resection and chemotherapy. Although it is a rare lesion, oral leiomyosarcoma is an aggressive neoplasm, highlighting the importance of understanding its clinical and histopathological aspects in order to promote early diagnosis and treatment.
