Resumo: Leiomyoma is a rare benign tumor with smooth muscle differentiation, and its unusual morphology in the oral cavity can lead to diagnostic errors. This case report presents a 7-year-old female patient referred to the oral medicine service, where intraoral examination revealed a hypertrophic upper labial frenulum associated with localized gingival hyperplasia. The lesion was asymptomatic, with a color similar to the normal mucosa, firm consistency, and smooth surface. A frenectomy with complete excision of the lesion was performed, and the sample was sent for histopathological analysis with a presumptive diagnosis of gingival hyperplasia. The histopathological analysis confirmed leiomyoma and, for academic purposes, an immunohistochemical assay was performed showing: SMA+, Desmin+, Caldesmon+ in muscle cells. Laser therapy with red laser at 2.0J per point every 24 hours was applied, totaling 4 sessions for tissue remodeling, with 11 months of follow-up, no recurrence of the lesion was observed.
