Resumo: Objective: This paper aims to present clinical data on tuberculosis (TB), and diagnostic methods. In this context, TB will be considered as diagnostic hypothesis in oral lesions.Study Design:This study was carried out in the form of a narrative review, using research into scientific articles published over the last 17 years in the main bibliographic databases. Results: Cases of spread to the oral cavity have been rare and can occur through blood, lymphatic system, and autoinoculation through expectoration. In order to have a diagnosis, it is necessary to go beyond the clinical evaluation and carry out other complementary tests, in which histological analysis is the most used for extrapulmonary diagnoses. Oral TB lesions present themselves as ulcerative lesions, with the following differential diagnostic hypotheses: syphilis, ulcerative lichen planus, carcinoma, and aphthous stomatitis. Conclusion: It can be concluded that the TB oral manifestations can be complex to diagnose them due to their clinical characteristics, the need for more than one diagnostic test and false negative results. The fact that TB is not very common contributes to misinformation about its occurrence and, although the lesions are rare, they should be included in the differential diagnosis.
