Resumo: A 35-year-old male patient presented with a two-month history of swelling in the right retromolar region and a previous history of endodontic treatment on the right second molar. Panoramic radiography revealed a poorly defined multilocular osteolytic lesion involving the body and ramus of the mandible. Cone-beam computed tomography showed involvement of the mandibular canal and disruption of the cortical bone. Incisional biopsy revealed a spindle cell neoplasm, with immunohistochemical positivity for EMA, vimentin, and CD34, confirming the diagnosis of a primary extracranial meningioma. Initial treatment involved partial mandibular resection and reconstruction with an iliac crest bone graft and locking plates. In 2019, a recurrence occurred, requiring further resection with removal of the periosteum and inferior alveolar nerve, while preserving the lingual nerve, followed by reconstruction with a vascularized fibula flap. As of 2025, the patient remains under clinical follow-up with no signs of recurrence.
