Resumo: Introduction: Chronic kidney disease (CKD) has a direct impact on bone remodeling and mineralization. Radiomorphometric indices (RIs) evaluated on panoramic radiographs (PRs) can identify images suggestive of decreased bone mineral density (BMD). Objective: To identify, through PRs, the prevalence of RI classifications that indicate decreased BMD and to verify whether there is an association with blood test results. Study Design: A retrospective observational study was conducted using 151 PRs of patients with CKD (stages III, IV, and V). The Mandibular Cortical Index (MCI) and Mental Index (MI) were analyzed. The results were compared with serum levels from blood tests. Results: The majority of patients were female (57,6%), with a mean age of 50,4 years. A total of 120 patients (79,5%) presented an altered MCI (C2 and C3), and 94 (62,25%) had an MI below the reference value of 3 mm (p < 0,001). The association between MCI and biochemical markers was statistically significant for vitamin D (p = 0,01). None of the independent variables were correlated with the MI. Conclusion: The CKD patients evaluated showed RIs that differed from normal patterns.
