Título: CLINICOPATHOLOGICAL ANALYSIS OF "RING AROUND THE COLLAR" LEUKOPLAKIA

Resumo: Objective: Marginal gingival leukoplakias ("ring around the collar") are relatively uncommon oral lesions. They appear distinct from conventional leukoplakias due to their aggressive clinical behavior and possible association with proliferative verrucous leukoplakia (PVL). The current study aimed to assess the clinicopathological features of these lesions. Study design: A retrospective study was conducted on 32 individuals with "ring around the collar" leukoplakia. Clinical and histopathological data were collected. K-means clustering and principal component analysis were used to identify subgroups. Results: Lesions affected mostly patients with an average age of 60.4 years, equally distributed between genders. Most were multifocal (75%) and clinically homogeneous (84.4%). PVL was diagnosed in 75% of patients. Treatments included scalpel (n=3) and laser excision (n=8), or both modalities at different moments (n=2). Nineteen patients were only monitored. During a mean follow-up of 95.4 months, 11 recurrences (73.3%) occurred, and 3 lesions underwent malignant transformation. Hyperkeratosis was the most frequent histopathological feature. Conclusion: "Ring around the collar" leukoplakias may show clinicopathological heterogeneity and have a strong association with PVL. Long-term follow-up is recommended due to high risk of recurrence and possibility of malignant transformation.
