Resumo: Marginal gingival leukoplakia ("ring around the collar") might present a treatment challenge due to their critical anatomical site and high recurrence rates. This case series gathered information of 10 patients treated with high-power diode laser. Patients had a mean age of 64.4 years, with a female predominance (7 women, 3 men). Eight had multifocal lesions, being all homogeneous. Eight were diagnosed with proliferative verrucous leukoplakia. Two had previously been treated for gingival squamous cell carcinoma. Over a mean follow-up of 27.4 months, 7 lesions recurred after an average time of 8.6 months, though lesions were smaller and clinically less aggressive. No malignant transformation was observed. These findings suggest that high-power diode laser excision might be a more suitable option for lesion control, especially in aggressive cases which are commonly treated with radical modalities. Due to high recurrence risk, long-term follow-up is essential regardless of the treatment modality.
