Resumo: According to WHO, Ameloblastic Fibro-Odontoma (AFO) is a rare entity, and it's believed that the vast majority of lesions with a histological appearance of AFO will mature into Odontomas. The present work aims to report the case of an 8-year-old male patient who underwent a routine panoramic radiography due to delayed tooth eruption, revealing a radiolucent lesion in the mandible associated with an impacted tooth. The lesion was painless, with anterior mandibular swelling observed intraorally. A CT scan showed a hypodense mandibular image with hyperdense foci, suggesting Odontoma. An incisional biopsy was performed, and histopathological analysis initially diagnosed Ameloblastic Fibroma. During the excisional biopsy, a soft, whitish lesion associated with the crown of left mandibular canine and first premolar was observed, with the presence of denticles. After curettage and osteotomy, L-PRF membrane was placed to optimize bone repair. Final diagnosis of AFO was stablished. The patient remains under follow-up.
