Resumo: Verrucous carcinoma (VC) is a rare, low-grade, well-differentiated variant of squamous cell carcinoma. This entity has been associated with human papillomavirus (HPV) infection, poor oral hygiene and tobacco use. A 53-year-old female smoker presented to the dental service for evaluation of a rapidly evolving, asymptomatic, white lesion on the left lateral tongue, measuring 1.2 cm. The clinical hypothesis was leukoplakia or malignant epithelial neoplasia. After incisional biopsy, histopathological examination revealed marked hyperkeratosis with blunt projections of well-differentiated squamous epithelium, deep bulbous processes and pushing margin. The squamous cells had abundant cytoplasm and enlarged nuclei and minimal atypia. Mitoses were rare, and no tumor necrosis was observed. The underlying connective tissue displayed a dense infiltrate of chronic inflammatory cells and focal areas of epithelial microinvasion. The final diagnosis of VC with microinvasive areas was obtained. After surgical resection, the patient remains under follow-up, with no signs of recurrence.
