Resumo: The orthokeratinized odontogenic cyst (OOC) is a developmental cyst lined with orthokeratinized stratified squamous epithelium. Its multiple or bilateral occurrence is rare. A 25-year-old healthy male patient presented with an incidental radiographic finding during pre-extraction planning for the lower third molars. Imaging revealed two well-defined unilocular radiolucencies associated with the crowns of the partially erupted teeth 38 and 48. Surgical extraction of teeth 38 and 48 was performed, along with an excisional biopsy of the lesions. The biopsied material was sent for histopathological examination with differential diagnoses of odontogenic keratocyst and dentigerous cyst. Histopathological analysis revealed a cystic capsule composed of dense fibrous connective tissue, lined with stratified squamous epithelium of variable thickness, exhibiting a prominent granular layer and a thick keratinized layer composed of orthokeratin. The final diagnosis was bilateral OOC. The patient remains under follow-up with no signs of recurrence.
