Resumo: Cemento-ossifying fibroma (COF) is a benign odontogenic fibro-osseous neoplasm characterized by the production of osseous and cementum-like calcifications within a fibrous stroma. A 43-year-old female patient presented to a specialized care center with a painless swelling in the right hard palate and the right paranasal sulcus region, with an indeterminate history of growth. Computed tomography revealed a well-defined, unilocular hypodense lesion with hyperdense areas extending from tooth 11 to tooth 17 and involving the right maxillary sinus. Tooth displacement and root resorption were observed. An incisional biopsy was performed. Histopathological analysis revealed fragments of hypercellular dense connective tissue with abundant deposition of mineralized material, presenting as spheroids of osteoid or cementoid material randomly distributed throughout the stroma. The final diagnosis was COF. The patient was referred for surgical excision and remains under follow-up with no signs of recurrence.
