Resumo: Chronic hyperplastic candidiasis (CHC) is a distinctive clinical subtype of oral candidiasis, which presents as whitish oral plaques, frequently at the labial commissure. Nodular type, CHC is a rare subgroup, which can simulate clinically other infections, even neoplasms and malignancy. A 44-year-old female presented with a nodular lesion on the tongue three years ago. Medical history was noncontributory and infectious disease serology was negative. The main clinical hypothesis was condyloma acuminatum. After excisional biopsy, microscopy showed a florid epithelial proliferation and intense lymphoplasmacytic infiltrate infiltrating muscle bundles, along with foci of candidiasis. Immunohistochemistry revealed predominance of T cells over B cells, numerous intervening granulocytes and macrophages, hyperplasia of Largerhans cells, and foci of plasma cells. Nodular type, CHC is rare and diagnostically challenging, requiring histopathological confirmation. Stomatology follow-up appointments are essential for monitoring clinical progression.
