Resumo: A 42-year-old male patient presented with a two-month history of numbness in the posterior mandibular region. Clinical examination revealed mild swelling in the right mandibular body, with swelling of the vestibular sulcus. Imaging studies showed a poorly defined hypodense lesion, associated with cortical bone thinning and disruption. Suspecting of a malignant neoplasm, an incisional biopsy was performed. Histopathological examination revealed a proliferation of plasma cells in a poorly vascularized stroma. Immunohistochemical analysis was positive for CD79, CD56, Plasma Cell, MUM-1, and lambda light chain, with a proliferative index of 10%. CD138 expression and in situ hybridization for Epstein-Barr virus were negative. The initial diagnosis was solitary intraosseous plasmacytoma versus multiple myeloma. The patient was referred for oncologic staging. A PET-CT scan showed no additional lesions, confirming the diagnosis of solitary bone plasmacytoma. Treatment consisted of exclusive radiotherapy leading to complete resolution. The patient remains disease-free after one year of follow-up.
