Resumo: Mucoepidermoid carcinoma (MEC) is the most common malignant neoplasm of the salivary glands, however, its clear cell variant (cMEC) is infrequently reported. A 36-year-old woman presented with a well-circumscribed, painless, reddish nodule measuring 1.5 cm on the left side of the hard palate, with fibroelastic consistency and superficial ulceration. Cone-beam computed tomography revealed a hyperdense lesion without adjacent bone involvement. Based on the clinical presentation, the main diagnostic considerations included pleomorphic adenoma and mucoepidermoid carcinoma. An incisional biopsy was performed, and histopathological and immunohistochemical findings suggested an unspecified salivary gland adenocarcinoma. The patient was referred to an oncology center, where surgical excision was performed. Definitive histopathological evaluation lead to the diagnosis of cMEC. Postoperative management included rehabilitation with a prosthetic obturator, and spontaneous closure of a buccosinusal communication was observed. The patient remains under regular medical and dental follow-up, showing excellent clinical progress with no evidence of recurrence to date.
