Resumo: A 54-year-old female patient presented with a tongue nodule of one-year duration, reportedly developed after contact with a hot spoon. Her medical history was unremarkable, aside from the use of anxiolytics and antidepressants. Clinical examination revealed a 10-mm single, asymptomatic nodule on the dorsal tongue. The lesion had mucosa-like coloration, round shape, pedunculated base, and fibrous consistency. An excisional biopsy was performed for diagnostic and therapeutic purposes, with a clinical hypothesis of traumatic fibroma or granular cell tumor. Histopathological analysis revealed dense connective tissue with underlying lobules of mature adipocytes scattered among collagen fibers, diagnosed as a fibrolipoma. Sutures were removed after one week, with complete remission. Although fibrolipoma is the most common lipoma variant, its occurrence on the dorsal tongue is rare, likely due to the scarcity of adipocytes in this region. This case also highlights an uncommon presentation of fibrolipoma with a pedunculated and fibrous appearance.
