Resumo: Objective: This study evaluated the 3-year overall survival of patients diagnosed with head and neck squamous cell carcinoma before and during the COVID-19 pandemic, and analyzed the association with disease severity at diagnosis. Study design: Two groups were compared - pre-COVID-19 (March 11, 2019 - March 10, 2020; n=47) and COVID-19 (March 11, 2020 - March 10, 2021; n=45). Data were collected from medical records at a public referral hospital, and mortality information was obtained from the National Registry of Deaths. Results: The mean time from diagnosis to treatment was 94.5 (±55.9) days pre-pandemic and 80.8 (±47.0) days during the pandemic (p=0.264). Mean age was 62.3 (±10.6) and 60.3 (±7.5) years, respectively (p=0.300). More severe lymph node metastasis (N2/N3) was significantly higher in the COVID-19 group (71.11% vs. 39.13%, p=0.02). The 3-year overall survival was higher during the pandemic (77.78%) compared to pre-pandemic (55.32%) (p=0.023). Mortality was associated with advanced metastasis pre-pandemic (p=0.011) but not during the pandemic (p=0.135). Conclusions: Despite increased lymph node severity in the COVID-19 group, 3-year overall survival was not negatively affected. Shorter time between diagnosis to treatment and younger age in this group may have influenced the results. Further analyses will assess overall 5-year survival.
