Resumo: Desmoplastic fibroma is a rare, locally aggressive, benign bone tumor that accounts for approximately 0.1% of bone tumors. It is characterized by a high recurrence potential and predominantly affects long bones and the mandible, with an extremely low incidence in the condylar region. A 23-year-old female patient presented to the Oral and Maxillofacial Surgery and Traumatology Service for the extraction of impacted third molars. A panoramic radiographic examination revealed a radiolucent lesion in the neck of the left mandibular condyle. Therefore, a computed tomography scan was requested, which showed an osteolytic focus with posterior border involvement. The patient underwent an excision of the fibrous lesion and rigid internal fixation with a 2.0 mm system plate under general anesthesia. Histopathological examination confirmed the diagnosis of desmoplastic fibroma. Following appropriate treatment and diagnosis, the patient has exhibited no signs or symptoms of recurrence in a 5-year postoperative follow-up period.
