Resumo: Mature cystic teratomas in the oral cavity are exceptionally rare particularly in pediatric patients. Their embryological origin remains unclear, though recent genetic studies suggest sequestration of primordial germ cells along the midline during fetal development. This case report describes a 5-year-old girl with a painless, submucosal nodule located on the floor of the mouth, measuring approximately 3 cm. Imaging revealed an isodense lesion above the mylohyoid muscle, displacing the tongue superiorly. Surgical excision was performed and the histopathological examination showed a mature cystic teratoma. It was characterized by cystic cavities lined with diverse epithelial tissues, exuberant glial tissue, smooth muscle, cartilage, and psammoma bodies. Immunohistochemistry was positive for CK7, CK13, CK14, CK18, CK19, α-SMA, SOX1, S100, and negative for CDX2, CK20 TTF1 and Ki-67 and supported the diagnosis. In addition, FISH for MAML2 was negative, ruling out mucoepidermoid carcinoma. The patient remains recurrence-free after 1.5 years of follow-up.
