Resumo: Leukoplakia is characterized as a non-scrapable white plaque of unknown etiology, being considered a potentially malignant disease. Verruciform xanthoma (VX) is a rare lesion often affecting the oral mucosa, and microscopically characterized by the accumulation of foamy macrophages in the lamina propria. A 63-year-old female patient presented with an elongated, whitish, soft-consistency, and rough-surfaced lesion, as well as an isolated whitish verrucous lesion, on the lateral border of the tongue during an intraoral clinical examination. An incisional biopsy was performed, and the specimen was sent for histopathological examination, which displayed a diagnosis of severe epithelial dysplasia associated with VX. Immunohistochemistry performed to better characterize both lesions, demonstrated strong positivity for CD68 and CD163 in the XV areas. This case highlights that simultaneous presentation at the same location of intraoral white lesions is possible, being relevant a detailed histomorphological analysis, sometimes complemented by immunohistochemistry, for the correct diagnosis.
