Resumo: Buccal bifurcation cyst (BBC) is a rare inflammatory odontogenic cyst that typically affects the buccal region of the first or second mandibular molars in children. Herein, we report the case of a 12-year-old boy presenting with a one-month history of facial swelling. On extraoral examination, slight facial edema was noted on the right side. Intraorally, no visible alterations were observed. Panoramic radiography revealed a unilocular, well-defined radiolucent lesion involving the roots of the right first mandibular molar and the partially erupted right second mandibular molar. Additionally, a similar unilocular radiolucency was observed surrounding the partially erupted left second mandibular molar. An excisional biopsy was performed under local anesthesia at both sites, and the specimens were submitted for histopathological analysis. The diagnosis of BBC was confirmed for both lesions. The patient's postoperative recovery was uneventful. At the latest follow-up, successful eruption of both second mandibular molars was observed.
