Resumo: Paget's disease of bone is a chronic metabolic disorder characterized by abnormal bone remodeling, which can lead to pain, deformity, and enlargement of the affected bones. Herein, we report a case of a 49-year-old white female who sought care due to intense mandibular pain, persisting for approximately two months. On extraoral examination, a firm and painful swelling was noted in the basal region of the left mandible, with no notable intraoral findings. Imaging revealed an ill-defined radiolucent area in the region of the left lower premolars. Given the suspicion of a malignant lesion or osteomyelitis, an incisional biopsy was performed under local anesthesia. Histopathological examination showed replacement of normal bone with fibrous tissue, suggestive of Paget's disease. Laboratory tests, including serum calcium, phosphorus, and alkaline phosphatase, supported the diagnosis. The patient was counseled and referred to an orthopedist for further evaluation of potential systemic involvement and to initiate treatment.
